The Registration Fee is $50 which includes meals & lodging. If you're a UCSF member, you can pay by completing the Recharge Form below, Diabetes_Recharge_Authorization_Form.pdf. Please make sure to complete the Recharge Form by SEPTEMBER 15, 2007. You can fax it to Hy Le at 415/731-3612 or by inter-campus mail to Box 0534.
